Classes must be pre-booked online. 

 

All club members, parents and visitors must complete a Covid-19 Declaration Form PRIOR to entering the club.

This form must be sent back to your instructor filled in and signed BEFORE you are able to return to training. You can print, sign, take a picture and then send it back to your coach, or you can print, sign, scan and return it by e-mail. If any of the answers below change in the next 14 days, it is your responsibility to immediately inform your instructor and the club CV 19 officer.
